سكاي نيوز عربية / ترجمات – أبوظبي / بالفعل، بدأت التجارب السريرية على لقاحات لفيروس كورونا المستجد، فيما ستبدأ أخرى تقريبا، لكن يظل من الصعب تحديد موعد طرحه.
فحتى لو سارت التجارب على ما يرام، هناك العديد من العقبات قبل أن يصبح اللقاح متاحا في العالم.
وباتت كل عيون العالم منصبة نحو اللقاح المرتقب للفيروس، الذي أعلنته منظمة الصحة العالمية مؤخرا جائحة، علّه يقي البشر خطر المرض الذي أزهق أرواح الآلاف حول العالم.
وبحسب صحيفة “غارديان” البريطانية، فإن 35 شركة ومؤسسة أكاديمية في العالم تتنافس على إنتاج لقاح يستطيع درء خطر الفيروس المعروف أيضا باسم “كوفيد 19“.
وقالت الصحيفة إن 4 من هذه المؤسسات بدأت في اختبارات اللقاحات التي طورتها، ومنها شركة “موديرنا” الأميركية التي بدأت التجارب السريرية على البشر.
ومن جانبها، أعلنت مجموعة “سانوفي” الفرنسية أن عقار “بلاكنيل”، الدواء المضاد للملاريا الذي تنتجه، برهن عن نتائج “واعدة” في معالجة مرضى بفيروس كورونا المستجد.
التسلسل الوراثي
ويرجع الفضل في السرعة الكبيرة لوصول الأدوية لمرحلة الاختبارات البشرية، إلى الجهود الصينية في تعقب تسلسل المادة الوراثية لفيروس “كوفيد 19”.
وكانت الصين تقاسمت المعلومات مع آخرين بشأن تسلسل المادة الوراثية في مطلع يناير الماضي، الأمر الذي أتاح للباحثين حول العالم دراسة الفيروس وكيفية غزوه للخلايا البشرية وإصابة الضحايا بالمرض.
ويقول الرئيس التنفيذي لتحالف الابتكارات المعدة لمواجهة الأوبئة، ريتشارد هاتشيت، إن سرعة “الشركات في إنتاج لقاح مضاد لفيروس كورونا تعتمد على استثمارهم وفهم كيفية تطوير اللقاحات السابقة ضد فيروسات عائلة كورونا السابقة”.
ويقود هذا التحالف، وهو عبارة عن منظمة غير ربحية مقرها أوسلو، الجهود من أجل تمويل وتنسيق الجهود لتطوير أحد لقاحات فيروس كورونا.
وتسببت الفيروسات التاجية أو ما تعرف بعائلة كورونا في ظهور وباءين، الأول في الصين بين عامي 2002 و2003، وعرف بفيروس سارس الذي يؤدي إلى متلازمة الجهاز التنفسي الحاد، وفيروس كورونا أو متلازمة الشرق الأوسط قبل سنوات.
وفي كلتا الحالتين، بدأت اللقاحات في العمل متأخرا، أي بعدما تم احتواء تفشي الفيروس.
اللقاح من الماضي
تقول شركة “نوفافاكس” الأميركية، ومقرها ولاية ميريلاند، إنها ستعيد استخدام اللقاحين المعدين لهذين الفيروسين من أجل مواجهة فيروس كورونا المستجد، قائلة إن لديها العديد من المتطوعين الذين يبدون استعدادهم لخوض التجارب السريرية.
ويتشارك فيروس سارس بنحو 80 إلى 90 بالمئة من المادة الوراثية المسببة للفيروس الذي تسبب بمرض “كوفيد-19″، ويعمل تقريبا بالطريقة ذاتها على مهاجمة سطح الخلايا البشرية ثم النفاذ إلى داخلها، خاصة في الجهاز التنفسي، حيث يبدأ الفيروس في إعادة إتناج نفسه بكميات كبيرة، ويخرج من الخلية بعد أن يقتلها.
وتعمل اللقاحات التي يجري تطويرها الآن طبقا لنفس المبدأ الأساسي، وهو تقديم جزء أو كل العوامل المؤدية إلى المرض إلى جهاز المناعة البشرية، على شكل حقنة بجرعة منخفضة، وذلك لتحفيز النظام على إنتاج أجسام مضادة للمرض.
3 مراحل ضرورية
ورغم أن التجاربة السريرية تبدو مرحلة متقدمة بالنسبة إلى كثيرين، فإنها ليست سوى البداية، طبقا لصحيفة “غارديان”، التي تقول إن التجارب السريرية التي تمت ليست سوى نقطة أولية من أجل الحصول على الموافقة التنظيمية، التي تتم على 3 مراحل.
وتشمل المرحلة الأولى اختبار اللقاح على بضع عشرات من المتطوعين الأصحاء، لرصد آثار اللقاح الإيجابية والسلبية.
أما المرحلة الثانية، فتشمل تجارب على مئات الأشخاص في مناطق عدة من العالم، خاصة تلك المتضررة من الفيروس، ودراسة النتائج بعناية تامة.
وبعد ذلك، يتم الانتقال للممرحلة الثالثة، وهي اختبار اللقاح على آلاف المتطوعين حول العالم.
وبالطبع خلال هذه المراحل، يجري استبعاد العديد من اللقاحات بعدما يتأكد مثلا أنها غير فعالة أو لها أعراض جانبية مؤذية، كما يقول بروس غلين، الذي يدير برنامج التحصين البشري بمؤسسة “سابين” للقاحات في واشنطن.
بعد 18 شهرا
ويقول أستاذ الأمراض المعدية في كلية لندن للصحة وطب المناطق الحارة أنيليس سميث، إن العمل على إنتاج لقاح يسير بسرعة كبيرة، لكن سيعترضه الكثير من العقبات.
وأضاف: “مثل معظم اختصاصيي اللقاحات، لا اعتقد أنه سيكون جاهزا قبل 18 شهرا”.
وكان الأستاذ الجامعي يعلق على محاولة الرئيس الأميركي دونالد ترامب إقرار اللقاح قبل نوفمبر المقبل، موعد الانتخابات الرئاسية في الولايات المتحدة.
وليس هذا فحسب، فبمجرد إقرار الدواء من قبل السلطات الصحية، سيكون هناك تحد أمام شركات الأدوية هو إنتاج كميات كبيرة منه تكفي الطلب العالمي عليه.
وبحسب “غارديان”، فإن الكثير من هذه الشركات ليست لديها القدرة الإنتاجية اللازمة لمواكبة هذا الطلب.
